  • Are you over age 40?
  • Are you overweight?
  • Do you have a family member with Diabetes?
  • Do you have high blood pressure?
  • Do you exercise fewer than three times per week? 
  • Have you ever had gestational diabetes?

Diabetes can affect not only your blood sugar, but also your heart, eyes, kidneys, and feet. In addition, related medications and health problems can be expensive and difficult to manage. 

One in three people have prediabetes – and many don’t know it.

Heading into our second year, Streu’s Diabetes Prevention Program continues to help people lower their risk of developing diabetes. Our lifestyle change program is a 16-week course followed by monthly check-ins for the rest of the year.

This program is available to all community members. You do not need to be a current patient.

Those who have been diagnosed with diabetes by their doctor are not eligible.
