Responsibilities
- Dispense medications accurately and efficiently within an LTC pharmacy setting, ensuring adherence to all legal and safety standards.
- Conduct thorough patient assessments to develop personalized medication plans that optimize therapeutic outcomes.
- Provide clinical pharmacy services including medication reviews, drug interaction analysis, and medical management consultations for residents.
- Collaborate with healthcare providers to develop and modify treatment plans, including pain management, immunizations, and pediatrics as needed.
- Compounds medications using standard formulas and processes such as weighing, measuring and mixing ingredients.
- Utilize pharmacy software, EMR (Electronic Medical Records), and EHR (Electronic Health Records) systems to document patient care activities accurately.
- Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times.
- Support pharmacy technician staff by providing guidance on medication preparation, inventory control, and medication administration procedures
- Engage in patient care activities such as immunizations, medication administration education, and acute care interventions within the LTC environment.
Education Requirements
- WI State Pharmacist License
- CPR Certified
- Immunization Administration Certificate
