Long-Term Care Pharmacist

Responsibilities

  • Dispense medications accurately and efficiently within an LTC pharmacy setting, ensuring adherence to all legal and safety standards.
  • Conduct thorough patient assessments to develop personalized medication plans that optimize therapeutic outcomes.
  • Provide clinical pharmacy services including medication reviews, drug interaction analysis, and medical management consultations for residents.
  • Collaborate with healthcare providers to develop and modify treatment plans, including pain management, immunizations, and pediatrics as needed.
  • Compounds medications using standard formulas and processes such as weighing, measuring and mixing ingredients.
  • Utilize pharmacy software, EMR (Electronic Medical Records), and EHR (Electronic Health Records) systems to document patient care activities accurately.
  • Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times.
  • Support pharmacy technician staff by providing guidance on medication preparation, inventory control, and medication administration procedures
  • Engage in patient care activities such as immunizations, medication administration education, and acute care interventions within the LTC environment.

Education Requirements

  • WI State Pharmacist License
  • CPR Certified
  • Immunization Administration Certificate
