Long-term care pharmacy services are the solution to simplifying your medication regimen and easing your mind when it comes to taking the right medications at the right time. Streu’s has been providing long-term care pharmacy services for decades and is now filling the need in Northeast Wisconsin and beyond for a reliable, quality and caring long-term care pharmacist who is an integral part of your health care team.
Do I Qualify?
Individuals may qualify for Long-Term Care at Home (LTCaH) pharmacy services if they meet any of the following criteria:
- Have limitations in at least two activities of daily living. These can include activities such as bathing, dressing, eating, toileting, transferring and walking, or more complex activities such as managing medications, preparing meals, handling finances, doing housework, shopping and using transportation.
- Take multiple medications.
- Live with three or more chronic health conditions.
- Would make it difficult to pick-up medication in-person at a traditional pharmacy
- Qualify for a Home and Community Based Services or another waiver program.
