Long-Term Care at Home (LTCaH) pharmacy services are designed to provide the same level of coordinated, high-quality medication management that residents receive in long-term care facilities — but delivered directly in the home setting.

These services help individuals with complex medical needs to safely manage their medications, reduce hospital visits and improve overall health outcomes while allowing them to remain comfortably at home.
By minimizing adverse drug interactions and supporting proper medication use, LTCaH pharmacy services can help lower emergency department visits, reduce healthcare costs and enhance quality of life for people living with disabilities or multiple chronic conditions.
Services include:
- Comprehensive medication regimen reviews and provider coordination.
- Adherence monitoring and medication technology support.
- Care coordination among healthcare providers.
- Custom medication dispensing and specialized packaging.
- Reliable delivery directly to the patient’s home.


Eligibility
Individuals may qualify for LTCaH pharmacy services if they meet any of the following criteria:
- Have limitations in at least two activities of daily living. These can include activities such as bathing, dressing, eating, toileting, transferring and walking, or more complex activities such as managing medications, preparing meals, handling finances, doing housework, shopping and using transportation.
- Take multiple medications.
- Live with three or more chronic health conditions.
- Have a situation that would make it difficult to visit a traditional pharmacy to pick-up medication
- Qualify for a Home and Community Based Services or another waiver program.
